We are seeking an experienced and driven Home Health & Hospice Marketer / Community Liaison who understands the healthcare referral market, has established relationships with referral sources, and can help us grow our census.

Our primary focus is growing commercial-insurance home health referrals and admissions. Hospice marketing experience is a strong plus.

The Home Health & Hospice Marketer / Community Liaison will develop and maintain strong relationships with physicians, nurses, case managers, hospitals, skilled nursing facilities, rehabilitation centers, assisted living communities, discharge planners, and other referral sources to generate qualified referrals and admissions.

The ideal candidate is a self-motivated healthcare marketing professional with an existing book of business
, established referral relationships, and a proven ability to develop territories and consistently generate admissions.

This position is ideal for someone who can hit the ground running, leverage existing relationships, develop new referral accounts, and produce measurable results.

• Develop and maintain strong relationships with physicians, nurses, case managers, hospitals, SNFs, rehabilitation facilities, assisted living communities, discharge planners, and other referral sources.
• Generate qualified commercial-insurance home health referrals and admissions

• Promote and generate appropriate hospice referrals and admissions

• Identify, establish, and develop new referral accounts and business opportunities.
• Leverage an existing book of business while continuously expanding referral relationships.
• Develop and grow designated territories throughout Katy, Houston, and surrounding areas.
• Conduct regular facility and physician office visits, presentations, educational sessions, and community outreach.
• Educate referral sources and community partners about A Hug Away Healthcare's home health and hospice services.
• Represent the agency at networking events, healthcare meetings, community events, and professional organizations.
• Maintain consistent communication and follow-up with referral sources.
• Work closely with the clinical and intake teams to facilitate timely referral evaluation and admissions.
• Maintain strong communication and follow-up to minimize service failures and ensure referral-source satisfaction.
• Meet or exceed established admission, referral, and productivity goals.
• Monitor market trends, competitor activity, and opportunities for business growth.
• Maintain accurate records of marketing activities, referral sources, visits, contacts, and admissions.
• Submit required marketing and activity reports to agency leadership.
• Perform additional business development and community outreach duties as assigned.

• Minimum of 2 years of experience in home health marketing, healthcare liaison, business development, or a similar healthcare marketing role.
• Home health marketing experience strongly preferred.
• Hospice marketing or liaison experience is a major plus.
• Established relationships with physicians, nurses, case managers, discharge planners, hospitals, SNFs, assisted living communities, and other healthcare referral sources.
• Existing book of business and established referral relationships strongly preferred.
• Proven ability to meet and exceed referral, admission, or sales quotas.
• Strong understanding of the home health referral and admission process

• Understanding of the hospice industry and referral process is a plus.
• Knowledge of the Katy, Houston, and surrounding healthcare markets.
