- Board Certified/Board Eligible (Urology)
- REQUIRED, NOTE AT NAME CLEAR. - Active NE license
- HIGHLY PREFERRED - Clean Malpractice
- HIGHLY PREFERRED - Availability of dates in the job description
- REQUIRED, NOTE AT NAME CLEAR. - Case logs are required
- REQUIRED, INCLUDE AT PRESENTATION. - Disclose if the provider has worked at any other CHI/Dignity facility
- REQUIRED, NOTE AT NAME CLEAR. - COVID VACCINATED
- REQUIRED, NOTE AT NAME CLEAR.
