We provide personalized home health and hospice services designed to help patients improve mobility, regain function, and maintain independence in the comfort of their own homes.
As a Home Health Physical Therapist
, you'll have the opportunity to make a meaningful impact through one-on-one patient care. Working closely with patients, families, and an interdisciplinary care team, you'll develop and implement individualized treatment plans that promote safety, independence, and quality of life.
We are currently seeking coverage in:
- Dawson County
- Phelps County
Qualified candidates will possess:
- Current Physical Therapist licensure
- Strong clinical assessment and treatment skills
- Excellent communication and interpersonal abilities
- A positive, caring, and professional attitude

- High personal integrity and commitment to service excellence
