Role:
This position will provide casework and support to residents of Connective’s programs to enhance their quality of life, to ensure the orderly and safe operation of the facility, and to maintain the goals of the counselling and/or rehabilitative plans.

STATUS AND CLASSIFICATION

  • Unionized; this position requires union membership. This position is open to applicants of all genders.

HOURS

  • Shift

    Hours:

    Morning 0800 - 1600, 0830 - 1630;
    Evening 1600 - 0000, 1630 - 0030;
    Overnight 0000 - 0800, 0030 - 0830
  • Casual employees must be available for at least 21 shifts per month, of which seven are overnight shifts.
  • If you are working full-time, or if you are a full-time student, we ask you for 15 shifts of availability per month of which five are overnight shifts (subject to employer or enrollment proof).

REPORTING AND WORKING RELATIONSHIPS

  • Reports to and is accountable to the Program Manager or their designate
  • Participates as a member of the Connective services team to ensure quality and continuity of services to clients
  • Liaises with staff and volunteers of community and government agencies as required
  • Liaises with Connective staff as necessary
  • Accepts clinical direction from the care team
  • Participates as a member of the Connective staff team

KEY ACCOUNTABILITIES

  • Participates in the assessment, goal setting, and program planning for individuals living semi-independently. Documents and implements the plan and provides input into the evaluation of the program.
  • Evaluates client needs and develops short-term plans to meet such needs with the active participation of residents and their families.
  • Works with staff, community service providers, and government/non-government organizations to inform the development and implementation of individual support plans aligned with harm reduction and Connective philosophies.
  • Assists residents to function more independently in their own homes and in the community; supports daily living skills, social skills, and behaviour management; facilitates activities such as grooming, basic cooking, money management, shopping, safety, and pet care; supports physical, recreational, educational, social, and vocational activities.
  • Recognizes and manages potential emergency situations to minimize harm.
  • Administers medication to residents in accordance with policy.
  • Maintains health and safety standards; oversees building as required to mitigate conflict and ensure safety.
  • Completes routine property checks and documents progress; provides feedback to residents and families.
  • Accompanies/transports clients to activities and appointments.
  • Maintains written/verbal reports on clients’ daily activities and progress; ensures documentation is complete and accurate.
  • Identifies community services to meet clients’ needs; liaises with agencies and professionals.
  • Monitors facility operations with residence staff, including cleaning, inventory, and food services; participates in meal planning and preparation; involves residents in these processes.
  • Participates in team meetings and other duties as negotiated.
