In this role, you will collaborate with an inter-professional team to provide insightful assessments and implement comprehensive care plans. You will play a critical role in ensuring residents receive the utmost care while also participating in their transition and support processes. Your leadership will shine through your daily interactions and care delivery.
Key Responsibilities:
• Conduct and document residents' clinical assessments
• Develop and assess care/service plans actively
• Participate in resident care conference discussions
• Administer medications and document treatments correctly
• Support the Director of Care in resident transitions
Requirements:
• Diploma in Practical Nursing with registration in good standing
• 3+ years’ experience caring for seniors
• Updated CPR certification required
• Familiarity with Microsoft applications
• Proven leadership and mentorship capabilities
