Wages : $49.20 - $63.47 per hour

Non-Union Associate Care Coordinators

Collaborate with a multi-disciplinary team to plan, assess, and deliver interventions that support residents' rehabilitation. Work closely with physicians, residents, families, and agencies to ensure safe, quality care focused on customer satisfaction and continual improvement.

Serve as a mentor and role model, facilitating strong leadership, team building, change, and resource management. Demonstrate clinical competency, problem-solving, decision-making, and conflict-resolution skills.

Graduate of a recognized school of Nursing and actively registered as a Registered Nurse. A BScN and Gerontology certificate are preferred. Current CPR and First Aid certifications required. Experience working with the elderly and disabled. Proficient computer skills.

• A current and clear Criminal Records Check Form, and if applicable a Vulnerable Sector Check obtained no more than 90 calendar days prior to hire date.
• A tuberculosis screening completed no more than 12 months prior to hire date.
