Salary Range: $32.84 To $44.96 Hourly

This position is covered under the HEABC wage levelling. Please note the wage is currently being adjusted and topped up by the government and the displayed wage is therefore reflective of such initiative.

We are pleased to offer a $10,000 signing bonus and up to $3300 in relocation assistance for this full-time permanent role at this community. Certain conditions apply – only available for new hires or returning employees who have not worked at any Retirement Concepts communities in the past 6 months.

The Licensed Practical Nurse (LPN) reports to the Director of Care and/or designate. This position isthe role model and team leader to an assigned comunity, and responsible for coordinating care delivery of the persons served. The LPN performs duties and monitorsdepartmental activities in accordance with their scope of practice, established standards, regulations, policies and procedures to maximize individualized and resident-centered care.

Include but not limited to:

• Be an engaged collaborative team player, developing and maintaining professional relationships that support and strengthen the organizational culture and purpose.
• Assesses residents’ needs; implements and evaluates individualized resident-centered care plans.
• Ensures residents rights are observed and adheres to aholistic philosophy of wellness.
• Liaises with physicians, community agency representatives, families, visitors, volunteers, other disciplines and departments to ensure quality resident-centred care is delivered.
