Age Care is seeking an experienced Licensed Practical Nurse to work under the Director of Care. You will complete resident assessments, update care plans, and actively participate in care conferences. Effective communication and collaboration with the care team are key to ensuring residents receive optimal care.

Key Responsibilities:

• Provide planned, patient-oriented nursing care

• Complete and update resident clinical assessments

• Implement and evaluate care plans as needed

• Assist with resident care conferences scheduling

• Administer medications and document care accurately

Requirements:

• Practical Nurse Diploma and current registration

• 3+ years of experience in seniors’ health

• Current CPR certification

• Strong proficiency in Microsoft Office

• Demonstrated leadership and mentorship abilities
