The Salvation Army is seeking an engaged individual to fulfill the responsibilities of a Case Worker for the Pathway of Hope initiative. This role involves connecting with participants, providing integrated case management, and delivering spiritual care through a collaborative approach. You will work closely with diverse community members to promote their goals and improve their circumstances.
Key Responsibilities:
• Meet participants through in-person and video conferencing
• Serve as a vital link within the Pathway of Hope team
• Coordinate community resources and referrals
• Maintain detailed records of services provided
• Facilitate training and outreach initiatives
Requirements:
• Two years of casework experience in social services
• Community College Diploma in human services
• Strong relationship-building skills
• Knowledge of community resources
• Valid Ontario Class “G” driver’s license
