As a Community Support Worker, you will participate in assessments and goal-setting for participants with diverse needs.

Your role involves implementing Support Plans and providing ongoing assistance in daily living skills, ensuring participants can thrive in their communities. Safety and well-being will be at the forefront of your responsibilities.

Key Responsibilities:

• Assess needs and develop individualized support strategies

• Facilitate social and life skills development activities

• Monitor safety and manage emergency situations

• Document participant progress and maintain accurate files

• Transport participants to events and activities

Requirements:

• Diploma in a related human/social service field

• One year of recent experience or equivalent training

• Must obtain a Class 4 driver’s license

• Successful completion of criminal record check

• Passion for community-focused, inclusive support
