As part of a vibrant team, your mission as a Community Support Worker will involve promoting health and safety for people with disabilities. Your responsibilities will include fostering relationships within the community, advocating for supported persons' rights, and assisting with their daily living activities. Expect to collaborate diligently within a multidisciplinary team to deliver high standards of support and care.

Key Responsibilities:

• Respect and advocate for supported persons’ rights

• Empower individuals to integrate into their community

• Implement and document Personal Service Plans

• Foster relationships with families and professionals

• Record daily activity and personal health information

Requirements:

• Grade 12 education and Community Support Worker Diploma

• Minimum 2 years in community social services

• Current First Aid/CPR and Food Safe certificates

• Effective verbal and non-verbal communication skills

• Criminal record clearance and TB test
