OverviewWe’re seeking compassionate and experienced RN Case Managers to join our team and deliver high-quality, culturally sensitive care using the Dene’ Model of Care. This role focuses on assessing, planning, and coordinating comprehensive healthcare services for assigned panels, ensuring continuity of care and improved health outcomes.
Day-to-Day- Assess healthcare needs of assigned panels using the Dene’ Model of Care and management tools
- Prioritize and advocate for patient needs
- Utilize Nurse Case Management Competency tool to guide independent practice
- Establish measures to address targeted health indicators with providers and wellness teams
- Collaborate with internal teams, Alaska Native Tribal Health System, and external healthcare facilities
- Use data to monitor patient panels and determine care needs
- Notify providers promptly of identified needs and follow up on specialist recommendations
- Educate patients and families to promote health and well-being
- Participate in staff meetings, continuing education, and quality improvement initiatives
- Build strong working relationships across teams and referral agencies
- Associate’s degree in Nursing
- RN license (active and in good standing)
- Basic Life Support (BLS) certification
- Minimum 3 years of RN experience
- Bachelor’s degree in Nursing
- Knowledge and experience working with cultural diversities

$39.65 - $51.00 per hour
Exact compensation may vary based on several factors, including skills, experience, and education.
