Hours:

9am to 5pm

About the Role:

As an Adults Social Worker, you will carry out independent assessment and support planning with the use of current legislation, theoretical models and best practice approaches.

• Independent assessment and Support Planning
• Work within the Transfer of Care hub for multi-disciplinary discussions and social work interventions
• Support and facilitate during hospital admission and hospital discharge
• Work in the community
• Undertake strength-based assessment and advocating support for people after discharge

• Education:
  
  Degree in Social Work (or equivalent)
• Licenses:
  Social Work England registered
• Experience:
  
  Minimum 3 years post qualified experience in direct employment with an English Local Authority

• Excellent communication and interpersonal skills
• Strong case management and problem-solving ability
• Resilience and empathy when working with complex cases
• Proactive team player who values safeguarding and empowerment
