The team at The James Cochrane Practice are looking for a dedicated Long Term Conditions(LTC) Nurse to join our friendly, patient centred practice. This is a rewarding role within our well established LTC team, where you'll support patients with a range of chronic conditions and play a key part in promoting long term health and wellbeing.

What you'll be doing
- Delivering high quality management of long term conditions
- Providing health promotion and preventative care
- Supporting personalised care plans and proactive patient reviews
- Working closely with our LTC specialist team and broader clinical / nursing team
- Contributing to a positive patient experience and high clinical standards
We are 2 site, dispensing practice in Kendal with around
17,500 patients keen to attract caring, motivated, and passionate colleagues.
We offer a wide range of enhanced services, plus Dermatology and ENT clinics.
We are a GP registrar and medical student training practice with an active frailty and care home support team. We are an accredited Veteran friendly practice, a Park Run practice and are proactive in women's and men's health.
We are part of an excellent Primary Care Network and supported with clinical services from a wide range of disciplines. We offer

Job responsibilitiesWhat we're looking for
- Experience in chronic disease management (e.g. Diabetes, Asthma, COPD, Hypertension, CVD) is an advantage, but training will be given to support your development into
LTC nursing - Confidence working independently as well as within a wider multidisciplinary team
- Strong clinical judgement and excellent organisational skills
- Aproactive, flexible approach, especially during busy periods
- Abilityto handle sensitive or confidential patient information professionally
- Ideally anurse prescriber with primary care experience (desirable but not essential)

Person Specification Qualifications- Hold current NMC registration
- Qualified Nurse Prescriber
- Nurse prescriber
- Diploma in area of speciality
- Experience of working in a primary care environment
- Experience of working as a practice nurse of community nurse
- You will be available to work extended hours clinics which currently include some evenings and Saturdays
