Location:

Westmorland General Hospital, Kendal. Salary: £57,528 – £64,750 per annum (Band 8a Annex 21).

Contract:

Permanent full‑time 37.5 h/week. Closing date: 30/07/2026 23:59.

An Advanced Clinical Practitioner (ACP) role based in Morecambe Bay, requiring travel across the three hospital sites and community sites. The post blends direct, high‑level clinical care with education, management and governance, with a focus on service development and change.

• Work closely with the Heart Failure team and multi‑disciplinary staff within the CCDE and MECCS directorates.
• Lead and develop practice to promote high quality service throughout the patient pathway.
• Provide specialised care for patients with heart failure – including clinics and home visits.
• Provide expert practice through leadership, consultation, clinical advice, teaching and support for staff.
• Work with clinical colleagues to give information, advice and support to patients with complex heart failure.
• Lead service development and continuous improvement activities.

The post operates across Furness General Hospital, Royal Lancaster Infirmary and Westmorland General Hospital, and community sites including Millom Hospital, GP practices, Queen Victoria Hospital and Ulverston Community Health Centre.

• Registered Nurse (registered on the relevant part of the NHS register) or Allied Health Professional.
• Masters Degree in Advanced Practice or working towards.
• Post‑registration course or qualification in specialist area (where applicable/available).
• Non‑medical prescriber.
