Rehab Without Walls Neuro Rehabilitation is seeking a compassionate and experienced LCSW or LCPC to join our interdisciplinary team dedicated to helping individuals with neurological injuries and conditions regain independence and reintegrate into their communities.

We provide neurorehabilitation services delivered in the home, community, workplace, or school environment. Our program is an affiliate of Bright Spring Health Services and aims to enhance functional capabilities and independence for patients with brain injury, spinal cord injury, or stroke/CVA.

• Experienced LCSW or LCPC passionate about providing counseling services that support an individual’s physical, mental, cognitive, and emotional adjustment through recovery.
• Provides therapeutic interventions and resource recommendations that address patient goals to achieve personal, career, and independent living objectives in an integrated setting.
• Values professional autonomy, teamwork, communication, and collaboration to maximize patient recovery.

• Performs comprehensive evaluations, assessing the social, emotional, and support needs of patients and their families.
• Sets measurable objectives formulated in conjunction with the rehab team.
• Assists the physician and other health team members in understanding significant social and emotional factors related to patient health problems.
• Instructs and counsels patients and families in coping with social and emotional responses connected with illnesses.
• Participates in care coordination activities and acts as a resource for other health team members in identifying and resolving patient needs.
• Identifies and assists the patient/family to utilize appropriate community resources to achieve measurable objectives.

• Master’s Degree in Clinical Social Work (LCSW).
• Current Licensed Clinical Social Worker licensure in the state of practice.
• Minimum one year experience in a rehabilitation setting serving individuals with traumatic brain injury, spinal cord injury, or acute diagnosis.
• CPR certification required.
• Ability to lift 50 pounds; lifting of up to 100 pounds may be required following safety procedures.
• Fine motor skills, visual acuity, and ability to walk/stand for extended periods.
• Physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently.

USD $40.00 - $46.00 / Hour
