We are looking for an Independent Optometrist to operate within Sam’s Club locations. The successful candidate will lead patient care, manage eye and vision services, and maintain overall health and well‑being of our members.

As an Independent Optometrist leasing space inside Sam’s Club, you will have the autonomy to set your practice schedule, patient appointments, fee schedule, and retain 100% of exam/professional fees.

• Establish your days of practice.
• Set hours of operation.
• Control patient schedule.
• Set your fee structure.
• Maintain complete ownership of exam and professional fees.
• Optionally, procure equipment or instrumentation and administrative or Opt-Tech support services if state permits.

• Doctor of Optometry (OD) from an accredited optometry school.
• Valid state optometry license.
• Ophthalmologist (MD or DO) from an accredited school.
• Valid state ophthalmology license.
• General Liability Certificate.
• Professional Liability Insurance Certificate.
• W‑9 form.

• Experience operating a practice in a roll‑up or independent setting.
• Proficiency with vision care software and electronic medical records.
• Strong business acumen and marketing skills.
