Always Compassionate Health is seeking experienced, dependable, and clinically skilled Registered Nurses (RNs) to provide one-on-one skilled nursing care to patients in their homes.
We are looking for professionals who demonstrate strong clinical judgment, excellent communication skills, and a commitment to patient safety. Ideal candidates are confident working independently, following individualized care plans, and delivering high-quality, compassionate nursing care.
Responsibilities- Provide skilled nursing care in accordance with physician orders, individualized care plans, and agency policies.
- Administer prescribed medications and treatments and monitor patient responses.
- Perform G-tube/PEG care, tracheostomy care, and suctioning as required.
- Provide ventilator care when required and appropriately trained.
- Assess patient conditions, recognize changes in health status, and report concerns promptly.
- Maintain accurate, timely clinical documentation.
- Educate patients and caregivers on care needs, treatments, and safety practices.
- Follow infection control standards and established patient safety protocols.
- Active New York State Registered Nurse (RN) license and current registration.
- Minimum of six months of home care nursing experience.
- Current CPR/BLS certification.
- Strong clinical assessment, documentation, and communication skills.
- Ability to work independently and follow individualized care plans.
- Reliable, professional, and committed to providing safe, high-quality patient care.
- Ability to meet the physical demands of the assigned case, including lifting up to 50 pounds when required, with appropriate assistance and equipment.
- Flexible weekday and weekend shifts, based on case availability.
- One-on-one patient care in a home setting.
