Chaplain/Spiritual Counselor – Visiting Nurse Association of SEMO Position Purpose
Responsible for providing pastoral care services to assigned patients/caregivers.
Position Requirements- Education: State/CMS regulations:
Be ordained, commissioned or credentialed according to the practice of an organized religious group; and have proof of completion of at least one CPE unit or a minimum of a bachelor's degree with an emphasis in counseling or related subjects. A chaplain/spiritual counselor who has been ordained, commissioned or credentialed and has a minimum of a bachelor's degree with an emphasis in counseling or related subjects must receive additional training within 90 days of hire.
That training must address common spiritual issues in death and dying, belief assessment skills, how to individualize care to patient beliefs, and varied spiritual practices or rituals. Proof of the training must be documented in the chaplain/spiritual counselor's file. A chaplain/spiritual counselor who has been ordained, commissioned or credentialed and has proof of completing at least one CPE unit does not require the additional training. - Certification: N/A
- Experience: Prefer at least one year's experience pastoring a church or previous association with a hospice.
- Ensures the delivery of pastoral care services to patients/caregivers assigned by supervisor.
- Contacts patient/caregiver within 5 working days of assignment.
- Provides spiritual and/or religious support as defined in the Care plan or requested by patient/caregiver.
- Serves as on-call Chaplain/Spiritual Counselor when scheduled.
- Contributes to continuity of care and effective communications between Hospice team members.
- Collaborates with patient/caregivers and Hospice team to ensure the delivery of safe and efficient quality Hospice services by participating in the IDG meetings.
- Reports patient status verbally or in writing to Hospice Director/IDG TCM if unable to be present at IDG meetings.
- Contacts team members if an issue arises between IDG meetings.
- Documents all patient/caregiver contacts on notes for the Medical Record within 1-2 working days of the contact.
- Participates in patient staffing and/or clinical meetings as scheduled.
- Participates in problem identification and possible solutions at department staff meetings.
- Participates in Hospice QAPI committee as assigned.
