Physical Therapist; PT

The Physical Therapist (PT) provides direct rehabilitation care to patients in a skilled nursing facility, focusing on improving functional abilities and quality of life within a patient-centered environment.

• Developing, assessing, and modifying treatment plans based on patient needs and goals
• Overseeing and evaluating care provided by PTAs, rehab aides, and students
• Documenting patient progress accurately and timely
• Utilizing therapy software effectively and providing excellent patient service
• Collaborating with interdisciplinary teams to ensure comprehensive care
• Participating in community health initiatives and maintaining professional relationships
• Adhering to safety, confidentiality, and compliance policies

• Graduation from an accredited physical therapy program (BSPT, MSPT, or DPT)
• Current licensure in the applicable state in good standing
• CPR certification (must be current)
• Strong communication, body mechanics, and decision-making skills
• Proficiency in Microsoft Word, Excel, and email
• Ability to work collaboratively within a team and demonstrate professionalism

• Implement and follow established treatment plans
• Set measurable, realistic goals for patient recovery
• Assist in patient evacuation and ensure safety protocols
• Maintain confidentiality and adhere to company policies
• Demonstrate dependable attendance and positive attitude
