Speech Language Pathologist; SLP

Role: The Speech Language Pathologist (SLP) provides direct speech therapy rehabilitation care to patients in a skilled nursing facility setting.
Key Duties: Deliver quality speech therapy, support patient progress, ensure compliance, and contribute to a positive, patient-centered environment.

• Assess, establish, and modify functional speech therapy goals.
• Implement and follow individualized treatment plans in accordance with regulations and standards.
• Chart progress accurately and timely, utilizing therapy software.
• Collaborate with interdisciplinary teams, patients, and families to meet patient needs.
• Supervise and evaluate Speech Language Pathology Clinical Fellows (CCCs) if applicable.
• Participate in community health projects and uphold patient safety, rights, and confidentiality.
• Maintain professional relationships and adhere to company policies and legal guidelines.
• Assist with patient evacuation and ensure dependable attendance.

• Graduate of an accredited speech therapy program (MS or MA).
• ASHA certification (CCC) or eligible for Clinical Fellowship Year (CFY); must maintain credentials.
• Current state license in good standing; CPR certification required.
• Preferred: 1+ year experience; new grads welcome.
• Proficient in Microsoft Word, Excel, and email communication.
• Strong decision-making, body mechanics, and teamwork skills.
• Knowledge of post-acute care regulations and procedures.
• Ability to demonstrate professionalism, confidentiality, and integrity.
