As a Registered Nurse (RN) in our community nursing role, you will visit patients in various settings to provide individualized, compassionate care that promotes comfort and independence in their homes.

• Use appropriate assessment tools and techniques to evaluate each patient’s individual needs.
• Execute and continuously monitor care plans to ensure quality outcomes.
• Engage patients collaboratively, building rapport and trust to support therapeutic relationships.
• Maintain and update professional patient records related to encounters and care plans.

• Registration with the CNO as a Registered Nurse in good standing.
• Valid driver’s license with access to a vehicle.
• Experience delivering community care is an asset.
• Reliability, flexibility, and willingness to travel within a designated area.
• Passion for making a difference in the community.
• Required on‑call coverage, including after hours, to ensure patient safety.

Sign‑on bonus up to $10,000.00 for qualified candidates.

Competitive base salary ranging from $32.66 to $37.39 per hour, with compensation based on skills, experience, and alignment with responsibilities.
