• Assess patients' individualized care needs regularly

Requirements:

• CNO registration as a Registered Nurse in good standing

• Valid driver's license and access to a vehicle

• Experience in community care is beneficial

• Flexibility and reliability in scheduling

• Willingness to provide on-call coverage
