Registered Nurse or Practical Nurse

Key Responsibilities:

• Assess patient needs using appropriate tools and techniques

• Execute and monitor individualized care plans

• Engage patients collaboratively to build rapport

• Maintain professional records of patient encounters

Requirements:

• Registration with the CNO as a Registered Nurse in good standing

• Valid driver’s license and vehicle access

• Experience in community care is an asset

• Flexibility and reliability for travel
