In this part-time role, you will visit patients in their homes, assisted by RNs for complex cases. Key Responsibilities:

• Conduct patient assessments using appropriate tools

• Execute and adjust individualized care plans

• Maintain professional patient records accurately

• Build rapport with patients through collaboration

• Provide safe nursing care within your scope of practice

Requirements:

• Registration with CNO as a Registered Practical Nurse

• Valid driver’s license and access to a vehicle

• Experience in community care preferred

• Flexibility and reliability for scheduled visits

• Willingness to travel within designated areas
