Key Responsibilities:

• Conduct community visits to address patient care needs

• Administer safe and ethical nursing practices

Requirements:

• Registered Practical Nurse credential with CNO

• Valid driver's license and vehicle access required

• Previous community care experience preferred

• Must be flexible for travel and on-call needs

• Strong desire to support vulnerable populations
