Overview

Register Practical Nurse (RPN) position located in Fort Frances, Ontario. Scheduled community visits to provide nursing care in patients’ homes, supported by RN for complex cases. Offer autonomy, independence, and work‑life balance.

• Participate in scheduled community visits to address patient needs within scope of practice.
• Provide ethical and safe nursing care.
• Use assessment tools to evaluate individual patient needs.
• Execute and continuously monitor care plans, adjusting as required.
• Engage patients collaboratively, building rapport and trust.
• Maintain and update professional patient records related to encounters and plans.

• Registration with the CNO as a Registered Practical Nurse in good standing.
• Valid driver’s license and access to a vehicle.
• Experience delivering community care is an asset.
• Reliability and flexibility.
• Willingness to travel within a designated area.
• Experience working with vulnerable populations.
• Passion for making a difference in the community.
• Availability for on‑call coverage, including after hours.
