As a Registered Practical Nurse, you will participate in scheduled visits to deliver ethical, safe nursing care in patients’ homes. Collaborate with RNs for complex cases while ensuring you meet individual patient needs using assessment tools. Engage with patients to build trust and adjust care plans as necessary for optimal support.
Key Responsibilities:
• Conduct community visits to assess patient needs
• Provide safe and ethical nursing care in homes
• Utilize assessment tools for individual evaluations
• Continuously monitor and adjust care plans
• Maintain professional patient records diligently
Requirements:
• Registration with CNO as a Registered Practical Nurse
• Valid driver's license and access to a vehicle
• Experience with community care preferred
• Willing to travel within designated areas
• Availability for on-call coverage after hours
