Position: Registered Practical Nurse with Community Focus
Location: Dryden

This position allows you to conduct scheduled home visits, supporting patients in various settings with complex care needs. With a focus on autonomy and independence, you will work collaboratively with a dedicated team to ensure patient comfort and safety in their homes while developing long-term therapeutic relationships.

Key Responsibilities:

• Perform community visits to assess patient care needs

• Ensure safe and ethical nursing practices are followed

• Utilize assessment tools for individualized patient care

• Monitor and adjust care plans as needed

• Engage with patients to foster trust and communication

Requirements:

• Registered Practical Nurse with CNO in good standing

• Experience in community care is an advantage

• Access to a vehicle and valid driver’s license

• Flexibility for on-call shifts and travel

• Passion for community service and patient support
