As a Registered Nurse or Registered Practical Nurse, you will conduct scheduled visits to support patients of all ages in various comfort settings. Key Responsibilities:
• Conduct scheduled community visits to meet patient needs
• Provide ethical and safe nursing care for diverse patients
• Assess patient needs using appropriate tools and techniques
• Monitor and adjust care plans as needed
• Build rapport and trust with patients for effective engagement
Requirements:
• Active CNO registration as RN or RPN in good standing
• Valid driver's license and access to a vehicle
• Experience in community care is an asset
• Reliability and willingness to travel for patient visits
• Experience with vulnerable populations preferred
