Clinical Supervisor - RN​/RPN in Home Care

Lead a dedicated team as a Clinical Supervisor with Para Med, ensuring high-quality care within the community. Utilize your nursing expertise to guide PSWs and HSWs while fostering patient relationships.

In this full-time position, the Clinical Supervisor is vital for overseeing caregivers and implementing clinical interventions. You will leverage your skillset in patient assessment, community nursing principles, and team leadership. This role encapsulates patient safety, mentoring, and collaborative problem-solving in home health care.

Key Responsibilities:

• Supervise PSWs and HSWs in their care practices

• Resolve community staff and patient concerns effectively

• Lead regular team meetings and one-on-ones

• Initiate and document patient safety investigations

• Collaborate on clinical documentation requirements

Requirements:

• Valid RN or RPN registration with the College of Nurses of Ontario

• Minimum 5 years of nursing experience

• Strong patient health assessment skills

• Proven leadership and management experience

• Valid driver’s license and reliable transportation
