Position: Professional Practice Leader for ACT Program

Key Responsibilities:

• Guide clinical mentorship and support ACT nurses

• Collaborate with community partners for efficient care

• Monitor clinical practices and ensure compliance

• Support program documentation and data management

• Participate in community outreach and education initiatives

Requirements:

• Registration in good standing with the College of Nurses of Ontario

• Bachelor’s degree in nursing required

• Minimum four years of relevant experience

• Knowledge of trauma-informed practices is essential

• Familiarity with community health resources and First Nations cultures
