Deliver community nursing care in Dryden with Para Med, using patient assessment and care planning skills. Scheduled visits support comfort at home.
As a Registered Nurse or Registered Practical Nurse, you will care for patients of all ages in varied community settings. You will work within your scope, supported by nurses for more complex patients. The position emphasizes autonomy, independence, and work-life balance.
Key Responsibilities:- Complete scheduled community visits within your scope
- Provide ethical and safe nursing care
- Assess individual patient needs using appropriate tools
- Monitor and adjust care plans as required
- Maintain professional patient encounter records
- Current CNO registration as RN or RPN
- Valid driver’s license and access to a vehicle
- Willingness to travel between patient residences
- Reliability and flexibility for community nursing
- Required after-hours on-call coverage
