Position: Social Worker with CCDC Team
Support the Centre for Complex Diabetes Care (CCDC) clients as a Social Worker, providing vital advocacy and psychosocial support as part of a multidisciplinary team.

Your role is crucial in improving patient outcomes and well-being.

At CCDC, Social Workers assess, advocate, and create tailored care strategies for patients dealing with diabetes. You’ll work closely with families and healthcare professionals to ensure comprehensive care, while also managing crises and addressing social health barriers. A solid background in psychosocial support and familiarity with community resources is essential for success in this role.

Key Responsibilities:

• Manage cases and support patients with diabetes

• Assess needs and collaborate on care plan development

• Provide crisis intervention and community resource referrals

• Promote healthy coping strategies and self-management

• Document all relevant patient interactions and care activities

Requirements:

• Degree in Social Work or equivalent education

• Registration or eligibility for OCSWSSW

• Eligible for Canadian Diabetes Educator Certification

• Strong assessment and case management skills

• Travel to remote locations may be necessary
