Crossroads Care Centers provided pay range

This range is provided by Crossroads Care Centers. Your actual pay will be based on your skills and experience — talk with your recruiter to learn more.

Base pay range

$42.00/hr - $45.00/hr

The primary purpose of your job position is to plan, organize, and supervise care of the residents in order to meet the total nursing needs of the facility’s residents and to provide medical treatment as prescribed by the attending physician. As PM Supervisor, you are delegated the responsibility and accountability to supervise, direct and instruct RNs, LPNs, CNAs, and RHs.

Receives report from the previous shift, direct or via 24 hour nursing report. Evaluates staffing, checks the daily schedule and staff attendance. Checks with the previous shift, Admissions Coordinator and Social Service department for any residents to be admitted or discharged for the day. Assists staff with admissions and discharges and checks for proper documentation and compliance.

Make frequent rounds to all residents and areas noting the following:

  • Appearance and condition of the resident and their room
  • Evaluation of the resident’s physical, mental, social, psychological, and religious needs/complaints.
  • Any breakdown in procedure is noted, immediate intervention is to follow by explaining procedure and rationale to the party involved.
  • Ensure restorative nursing techniques are being carried out.
  • In the bathing area, evaluate bathing procedures and the condition of the resident’s skin, scalp and nails.
  • Cleanliness of utility rooms, unnecessary supplies, tidiness, and locked cabinets.
  • Personal appearance of unit personnel.
  • Necessary supplies for treatment care, paper supplies, utensils, medical, and nursing supplies are well stocked. Empty rooms are clean and ready for new residents.
Assists the DON and ADON with infection control surveillance and tasks such as resident and staff immunizations as needed.

Monitors new staff (RNs, LPNs, CNAs) during their orientations and provides assistance as needed as well as completing the orientation check off list.

Make sure all restorative nursing techniques and procedures are being executed, understood, and documented. (i.e. ROM, B & B Retraining, ambulation, change of position, treatments, temperature, use of gait belts, training in self care, any cleaning duties that shift, etc.)

Informs DON of any pertinent changes or information concerning residents, their families, and/or staff members as those changes occur.

Needs to be aware of any emergency procedures related to natural or man made disasters (tornadoes, power failures, bomb threat, fire) and immediately informing the Executive Director, Administrator, and Maintenance Director of the campus.

Immediately informs Administrator, DON or designee about any significant incident/accident involving residents and/or staff (injuries requiring hospitalizations, allegations of resident abuse or neglect, resident elopement, violence at the work place).

Evaluate nursing staff under your supervision with the assistance of the DON and discusses evaluation with employee.

Cooperate fully with the Activity, Nursing Rehabilitation, Dietary, and Social Service departments so that the areas, along with the nursing department, may function together for the benefit of the resident.

Give a meaningful report to the oncoming shift including such pertinent information as new admissions, condition of critically ill patients, elevated temperatures, and any other significant knowledge necessary for continuity of care.

Conduct Nursing staff in-services per DON / ADON request.
