. This role is responsible for wound assessment, treatment, and supervising the daily nursing functions performed by the Nursing Assistants.
$39.00/hr - $43.00/hr
Job SummaryThe Certified Wound Care Nurse assesses, plans, implements, and evaluates wound care for residents, collaborates with the interdisciplinary team, and ensures documentation and care practices support optimal healing and regulatory compliance. In addition, the nurse provides quality nursing care and supervises the day‑to‑day nursing functions performed by Nursing Assistants.
Responsibilities- Perform comprehensive skin and wound assessments upon admission, weekly, and as needed.
- Stage pressure injuries accurately per NPIAP guidelines.
- Assist physicians in developing individualized wound care plans using evidence‑based treatment protocols.
- Perform and oversee wound cleansing, dressing changes, and advanced therapies.
- Evaluate wound progress and adjust treatment plans as needed.
- Conduct wound rounds with the facility wound doctor once per week.
- Implement preventive interventions (turning schedules, off‑loading, moisture management, nutrition referrals).
- Collaborate and communicate with providers, therapy, dietitians, and nursing leadership.
- Communicate wound status and changes promptly to physicians and families.
- Coordinate care with external wound specialists or consultants.
- Document wound assessments, measurements, progress notes, and treatment changes in the EMAR.
- Work closely with the Nursing Management Team.
- Be available Monday through Friday and on call one weekend per month.
- Oversee the assigned unit and assist floor nurses and CNAs when needed.
- Assist in the admission/discharge process.
- Complete accident/incident reports and follow up on investigations.
- Transcribe physician orders and document resident information accurately.
- Review medication orders for completeness and accuracy.
- Learn and adapt to the electronic medical record system.
- Prepare and administer medication as ordered by the physician.
- Participate in care planning processes and care conferences.
- Develop and maintain positive relationships with all disciplines to ensure resident needs are met.
- Participate in continuing education programs.
- Maintain current RN license in the state.
- Exercise tactful communication with personnel, residents, families, visitors, and government agencies.
- Registered Nursing License required.
- Must be wound certified.
- Licensed to practice as an RN in the state.
- Experience in wound care and seniority in nursing preferred.
