Humana/iCare is seeking an RN Care Manager to join the Family Care Partnership (FCP) program, serving Racine, Kenosha, Waukesha, Washington, Ozaukee, Walworth and Sheboygan counties. You will conduct nursing assessments and coordinate services in home/community settings as part of an Interdisciplinary Team.
Must hold Wisconsin RN license with 2+ years of related healthcare experience; BSN preferred. Travel up to 50%; reports to the Manager of Care Management; CST schedule.
