As a key member of the interdisciplinary care team, you will report to the Director, Professional Practice and Infection Prevention and Control. This role focuses on assessing health conditions and providing supportive and palliative care tailored to individual needs. Your contributions will enhance patient outcomes through collaboration and holistic approaches in a long-term care or hospice setting.
Key Responsibilities:
• Conduct comprehensive physical exams and health assessments
• Develop and implement individualized care plans
• Diagnose and manage both acute and chronic health conditions
• Prescribe medications and carry out minor procedures
• Collaborate with interdisciplinary teams for patient transitions
Requirements:
• Minimum 3 years of clinical experience as a Nurse Practitioner
• Master’s degree in nursing with NP certification
• Current registration with the College of Nurses of Ontario
• Strong clinical assessment and decision-making skills
• Knowledge of Ontario healthcare legislation and cultural safety principles
