Community Program Coordinator Health

Empower older adults with life-enriching programs as a Community Program Coordinator at Kensington Health. Focus on person-centered care, community engagement, and diverse programming in a supportive role.

In this role, you will coordinate therapeutic and recreational initiatives that enhance member wellbeing and social connection. Reporting to the Manager of Community Care, you'll engage with participants, adapt programs as needed, and work collaboratively with volunteers and community partners across multiple sites.

Key Responsibilities:

• Plan and facilitate therapeutic, educational, and wellness programs

• Monitor participant wellbeing and provide referrals as needed

• Maintain accurate attendance and documentation records

• Coordinate events, supplies, and room bookings

• Support volunteer orientation and day-to-day coordination

Requirements:

• Minimum two years in community services or health care

• Strong interpersonal and communication skills

• Experience with documentation and program coordination

• Diploma in relevant field and CPR certification

• Knowledge of community resources and referral pathways
