Kensington Health seeks a Social Worker to enhance quality of life in its 19-bed hospice. This role requires empathy and commitment, providing therapeutic interventions for end-of-life issues, grief, and loss. You'll organize family meetings and advocate for residents' rights while working collaboratively with an interdisciplinary team to ensure comprehensive support.

Key Responsibilities:

• Conduct psychosocial assessments for patients and families

• Develop individualized care plans with the care team

• Provide counseling on end-of-life issues and grief

• Facilitate family meetings and support group sessions

• Advocate for resident rights and preferences

Requirements:

• Master’s Degree in Social Work from an accredited institution

• Minimum of two years' experience in hospice care

• Proficient in Point Click Care and Microsoft Office

• Strong interpersonal and communication skills

• Licensed or eligible for licensure in Ontario
