Position: Social Worker for Hospice Services
Kensington Health invites applications for a skilled Social Worker dedicated to hospice services in Toronto. Provide vital psychosocial support to patients and families navigating end-of-life care.

The role focuses on delivering compassionate care within Kensington Hospice, the largest free-standing hospice in Toronto. You'll conduct comprehensive assessments, collaborate to develop care plans, and deliver therapeutic interventions. This position engages in family meetings and resource guidance while addressing emotional, social, and practical needs through an interdisciplinary approach.

Key Responsibilities:

• Evaluate emotional and practical needs through assessments

• Collaborate on care plans with the interdisciplinary team

• Offer support counseling for grief and loss challenges

• Coordinate family meetings to foster communication

• Advocate for individual cultural and ethical rights

Requirements:

• Master’s Degree in Social Work (MSW) required

• At least two years in community-based hospice care

• Familiarity with Point Click Care documentation

• Excellent communication and team collaboration skills

• Appropriate licensure as a Registered Social Worker
