Overview

Nurse Practitioner / Physician Assistant Primary Care with Eventus Whole Health. Eventus Whole Health delivers an integrated model of care to adults residing in skilled nursing and assisted living facilities through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants and in-house Support Staff.

Responsibilities
  • Provide primary-care services in accordance with Eventus Whole Health site-specific protocols and the standard of care for family medicine, geriatric medicine, and/or internal medicine as it pertains to long-term care facility work.
  • Review and sign MARs as requested by facilities; sign all orders specific for Primary Care while at the facility and/or within 24 hours if faxed or emailed, including phone orders, DME orders, Death Certificates, Plans of Care, PT/OT orders, hospice orders as appropriate.
  • Review and address labs within 24 hours of submission. Manage narcotic prescriptions for your facility’s patients per Eventus Whole Health Narcotic Rx Policy. Provide facilities with adequate time during each visit for paperwork and administrative duties.
  • Provide on-call triage duties per the on-call triage protocol policy. Adhere to the latest triage schedule as determined by Eventus Whole Health providers.
  • Complete documentation and submit billing records according to published charting and bill sheet submission policies; provide a hard copy of the electronic documentation within 48 hours of service for the patient’s facility medical record.
  • Maintain open and positive communications with facility staff and administration. Provide verbal and/or written guidance regarding medications and other caregiver information. Meet with the Administrator or designated facility care coordinator prior to the start of patient visits each day and prior to departure each day.
  • Adhere to Eventus Whole Health standards, policies, procedures, and all legal, regulatory, and ethical guidelines. Comply with governing health-care regulatory agencies.
  • Maintain patient confidentiality at all times, including appropriate use of cell phone, email, text messaging, patient charts, and EMR. Abide by HIPAA rules and regulations.
  • Consult with the clinical supervisor before refusing a resident, family, or facility request or recommendation regarding care.
Qualifications / Education
  • Masters degree in Nursing from an accredited institution and a current and valid medical license issued by the state of residence. All other certifications necessary to perform duties must be current and valid.
  • OR Graduated from a physician assistant program accredited by the National Commission on Accreditation of Allied Education Programs (NCAAP). Passed the national certification examination of the National Commission on Certification of Physician Assistants. Licensed as a Physician Assistant in the state of residence. Specialization in geriatric, family, or psychiatry desired.
