Social Services Designee

As a Social Services Designee, you will play a vital role in ensuring our patients receive the emotional and social support they need to thrive. Responsibilities- Conduct social support assessments to identify patients' emotional, social, and vocational needs
- Develop and implement individualized plans to address patients' needs and goals
- Collaborate with interdisciplinary teams to ensure comprehensive care and support
- Provide counseling and education to patients and their families on issues related to aging, disability, and chronic illness
- Monitor patients' progress and adjust plans as needed to ensure optimal outcomes
- Maintain accurate and thorough records of patient assessments, plans, and progress
- Bachelor's degree in Social Work, Psychology, or related field (n/a)
- 0 years of experience (minimum)
- Strong communication, interpersonal, and problem-solving skills
- Ability to work independently and as part of a team
- Compassion, empathy, and understanding of the needs of vulnerable populations
- Familiarity with social support services and community resources
