An exciting opportunity has arisen for a motivated and experienced

Senior Medicines Optimisation Technician to join theKCHFT-LMO Medicines Optimisation Team. This key role will support the delivery of the Medicines Optimisation Strategy, helping to meet the pharmaceutical care needs of the local population and improve patient outcomes across primary care services.

Working collaboratively with GP practices, Primary Care Networks (PCNs), prescribers, and wider healthcare professionals, you will play a pivotal role in promoting the safe, effective, and evidence-based use of medicines.

• Support prescribers and practice teams in the safe, effective, and cost-efficient use of medicines.
• Undertake medicines-related reviews, audits, and quality improvement initiatives.
• Analyse and utilise prescribing and patient data systems to identify opportunities for optimisation and improved patient care.
• Contribute to the delivery of medicines optimisation priorities, including QIPP (Quality, Innovation, Productivity and Prevention) programmes and medicines safety initiatives.
• Lead and support innovative projects that enhance medicines optimisation across GP practices and Primary Care Networks.

Provide mentoring, guidance, and professional support to pharmacy technicians and other members of the medicines optimisation team.

• Promote and encourage safe, evidence-based prescribing practices in line with national and local guidance.
• Work closely with multidisciplinary teams to improve patient outcomes, reduce medicines-related risks, and ensure high-quality pharmaceutical care.

Kent Community Health NHS Foundation Trust is due to integrate with Medway Community Healthcare on 1 October 2026. Following integration and to align our community services to the new Neighbourhood Model of care delivery, there will be a period of consultation which may result in changes affecting some services, teams or roles.
