What you will do
- Undertake Care Act assessments, reviews, and support planning using a strengths-based, person-centred approach.
- Facilitate safe and timely discharges from acute and community hospitals in line with the Discharge to Assess model.
- Work collaboratively with NHS colleagues, therapists, community providers, carers, and voluntary sector organisations to achieve positive discharge outcomes.
- Complete Mental Capacity Act assessments, make best interest decisions, and contribute to safeguarding enquiries as needed.
- Assess risks, balance individual choice with duty of care, and develop appropriate risk management plans.
Qualifications
- Degree in Social Work, CQSW, DipSW or an equivalent recognised qualification.
- Evidence of continuing professional development.
Registration & Compliance
- Current registration with Social Work England (SWE).
Experience
- Post-qualification experience within Adult Social Care.
- Experience working within hospital discharge, community social work, reablement, or integrated health and social care services.
