Patient Services Coordinator LPN Home Health at Center Well Home Health

Base pay range: $49,900.00/yr – $67,400.00/yr

• Manage patient schedules, including edits for agent absences and updates in the worker console.
• Initiate infection control forms and submit completed reports to HRD.
• Serve as backup during lunch hour and other busy times, field calls, and weekly case conferences; refer clinical questions as needed.
• Maintain client hospitalization log, enter coordination notes, and send electronic log to all office, field, and sales staff.
• Schedule all add‑ons and applicable orders, ensuring skilled nurse/injection visits are booked unless an aide supervisory visit is scheduled with the injection visit.
• Run scheduling reports, including Agent Summary Report and Missed Visits Done on Paper Report.
• Prepare weekly Agent Schedules; review for productivity/geographic issues and forward to Branch Director for approval before distribution.
• Verify visit paper notes in the scheduling console as needed.
• Assist with internal transfer of patients between branch offices.
• When clinical, receive lab reports, assess for normality, fax to doctor, route copies to Case Manager, and route to Medical Records Department; initiate Employee/Patient Infection Reports as required.
• When clinical, may be required to perform patient visits and/or participate in on‑call rotation.

• Licensed Professional Nurse or Licensed Vocational Nurse licensed in the state of practice.
• At least 1 year of home health experience.
• Preferred prior packet review/Quality Improvement experience.
• Preferred coding certification.
• Valid state driver’s license and automobile liability insurance.
• Must be currently licensed in the state of employment if applicable.
• Excellent communication, organizational, and multi‑assignment management skills.
