Behavioral Health Nurse LTC

- Address the mental, behavioral and physical health needs of clients on a weekly/biweekly and/or monthly basis.
- Complete in‑person nursing assessments as well as medication and chart review in long‑term care facilities (LTC).
- Educate caregivers and clients on their prescribed psychiatric and physical health medications and monitor for possible side effects.
- Care management duties including coordination of community services and correspondence with treatment team members, including physicians, pharmacies, and employment programs.
- Assist clients in building coping skills to address mood, stressors and improve communication, and in creating annual goals as it relates to their psychiatric diagnosis.
- Possibly assist clinic and residential needs for assigned locations.
- Help train and mentor peers.
- May need to cover more than one facility depending on company needs.
- Graduate of an accredited nursing program.
- LPN or RN license.
- Experience in psychiatric, mental and behavioral health services.
- Willingness to travel throughout assigned service area.
- Preferably has experience working in LTC facilities as well as Behavioral Health.
- CPR or course can be provided during training.
