Practice Info

• Value-based, full-risk Medicare Advantage model
• Patient panel capped at 450 patients
• Focus on prevention, care coordination, and behavior change
• Dedicated care teams (MA, Care Coordinator, Care Facilitator)
• Emphasis on quality, patient experience, and physician wellness
• Appointment utilization (18-21 slots per day)

Responsibilities

• Provide comprehensive primary care to older adult patients
• Maintain full accountability for clinical outcomes and patient experience
• Serve as a health coach, guiding lifestyle and long-term health improvement
• Coordinate specialty and hospital care while staying actively involved
• Lead and collaborate with an interdisciplinary care team
• Supervise and remain accountable for Advanced Practice Providers (APPs)
• Partner with medical and operational leadership to meet quality and cost goals

Compensation

• Salary: $250,000 - $306,714 (based on experience)

Qualifications

• MD or DO required
• Board Eligible or Board Certified in Internal Medicine, Family Medicine, or Geriatrics
• Active or eligible state medical license
• Active DEA
