Waiver Service Advisor at FIVCO Area Development District

Pay Range

$30,000.00/yr - $40,000.00/yr

General Summary

Under the supervision of the Waiver Supervisor, the Waiver Service Advisor will link clients to appropriate, comprehensive, and timely home or community-based services as identified in the plan of care. The role includes making home visits and phone calls as required.

General Duties And Responsibilities
  • Plan, refer, monitor, advocate, and follow the timeline of the assessor to obtain a renewed Level of Care and development of the Plan of Care for participants who are approved to recertify through the program annually.
  • Coordinate services and support from all agencies involved in providing services required by the Plan of Care.
  • Ensure that all service providers have a working knowledge of the Plan of Care.
  • Assist the participant in accessing needed services through referrals, linkages, etc.
  • Responsible for coordinating, arranging, and documenting client services provided.
  • Make a reasonable effort to secure and utilize informal support for clients and document reasonable effort in the case record.
  • Monitor clients by conducting a home visit in accordance with program guidelines and make telephone calls between home visits.
  • Enter all data into the reporting system in a timely manner.
  • Document in the case records each contact made with a client or on behalf of a client.
  • Explain necessary paperwork and assist with client complaints as needed.
  • Assist in monitoring the assigned waiver participant’s employee requirements if the client chooses the Participant Directed Services (PDS) route of waiver.
  • Assist with training participants, representatives and their employees on program requirements including identifying and reporting abuse, neglect, and exploitation, attendant care training, and use of the electronic verification system (if PDS).
  • Approve participant’s employees’ hours worked utilizing the electronic verification system (if PDS).
  • Assist in monitoring the participant’s service budget to ensure that there is no lapse in care or over usage of services.
  • Report any critical incidents to the cabinet utilizing the approved data system within 24 hours of discovery.
  • Complete required training.
  • Initiate corrective action plans as needed to ensure compliance of participant and/or providers.
  • Travel to required meetings and trainings, including overnight.
  • Perform any other duties as assigned.
Minimum Qualifications
  • Bachelor’s degree in social work/human services or related field.
  • Bachelor’s degree in any field not closely related AND one year of human services related experience OR
  • Associate degree in a behavioral science, social science, or closely related field AND two years human services related experience OR
  • Three years of human services related experience.

  • Experience as a case manager or in a related human services field

  • Paid professional experience with aging and/or disabled populations or programs as a Case Manager, a Rehabilitation Specialist or Health Specialist, and/or Social Services Coordinator
  • Assessment and care planning experience with clients
  • Experience in working directly with persons with intellectual, developmental, or other types of disabilities or mental illness
  • Work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention health, cultural competencies, inadequate housing.
  • Be a registered nurse (RN) currently licensed in Kentucky as defined in KRS 314.011(5) who has at least two (2) years of experience as a professional nurse in the field of aging or disabilities; or a master’s degree in a health or human services field from an accredited college or university.
