Position: Case Management/Support Broker (IDDS)
Location: Mount Sterling

Educational Requirements
  • Minimum of a Bachelor’s degree in Social Work, Education, Psychology, Special Education, or a Human Services field.
Position Overview

This is a hybrid position based in the Gateway region (Morehead, Mount Sterling, Owingsville, Frenchburg and West Liberty, KY). The Service Coordinator will assure the provision of appropriate services for children and adults with, or at risk for, intellectual and developmental disabilities.

Duties & Responsibilities
  • Determine the nature of referrals and assess the need for additional contacts or screenings.
  • Provide intake into the IDDS system.
  • Conduct home visits to identify family/individual priorities, resources, and concerns.
  • Facilitate, develop, review, and monitor person-centered plans on an ongoing basis.
  • Coordinate, monitor, and ensure timely delivery of services outlined in the person-centered plan.
  • Maintain thorough documentation and comply with all agency policies.
  • Perform other duties as assigned in the current job description.
Skills & Knowledge
  • Strong working knowledge of community resources and how to access them.
  • Ability to engage effectively with individuals and families regarding developmental disability concerns.
  • Proficiency in computer systems and electronic documentation.
  • Prior experience working with the IDD population is required.
Travel
  • Travel within the FIVCO region required.
  • Mileage reimbursement provided.
